What is Ohio Social Prescribing Collaborative?

The Ohio Social Prescribing Collaborative is a cross-sector coalition working to promote social prescribing in our state.  Social prescribing empowers providers to connect patients with community-based resources, such as arts, movement, service opportunities, and social engagements, to extend care beyond the clinic walls.

Why is Social Prescribing Important?

Social isolation, sedentary living, and disconnection from community are now recognized as serious health risks. Social prescribing gives clinicians a structured, evidence-based way to address what happens between appointments.
